Impact of stratified resection margin classification on local tumor control and survival in patients with oral squamous cell carcinoma

Abstract Background Guidelines recommended for resection of oral cancer define a free margin ≥5 mm as clear and safe (R0). This statement was questioned recently based on the assumption that different surgical margins may hold risk categories. The aim this study to investigate impact stratification survival outcome patients with cancer. Methods In cohort 753 patients, hazard ratio local recurrence‐free (LRFS), overall (OS), cancer‐specific (OCSS) were estimated R0 resection, close 1–4 mm, involved borders but frozen sections. Competing factors considered in statistical regression model. Results One hundred seventy‐three (23%) developed recurrence 316 (42%) died 5 follow‐up years. There gradual improvement LRFS, OCSS, OS increase margin. showed similar tendency. Conclusion Not all an R0cm status carry same impaired OS. Their develop is higher than those stratified management can be according presented results.
